Your sensor lights up when powered by USB but there's no sign of life on battery. Here's what to try:
First, insert a working a battery into the sensor. You may need to adjust the battery slot prongs to the middle so they hold the battery tighter. We found that some brands, like Panasonic for example, make their CR123 A batteries just a fraction of an inch shorter than the included battery, which could cause the battery to fall out of the slot once the mounting bracket is installed (due to the magnet pulling it out).
So once you insert a fresh battery into the tighter slot, exclude the sensor from your Z-Wave system and include it again making sure you triple click the Z-Wave button instead of pressing and holding the button. The latter method would enroll the sensor as an always-listening node, which would deplete the battery very quickly. The button needs to be pressed 3 times quickly at inclusion to have it work as a battery-powered Z-Wave sensor.
If the above didn't address the issue, double check that the battery itself is working and that it is positioned correctly in the compartment. Another thing that may help is performing a hard reset on the device using the steps shown here.
