Will the ZEN12 Motion Dimmer not include to your hub? Let's begin by excluding the unit.
Exclusion is known by many names: unpairing, disconnecting, or removing are all ways to refer to exclusion. This acts as a soft reset that can assist with a variety of configuration or connectivity issues. The ZEN56 doesn't need to be included in your system for it to be excluded successfully.

Put your Z-Wave hub into exclusion mode (see your controller's manual for details, as each brand is different)

Click the main button 3 times; all the LED's will blink magenta. The LED's will turn solid green for 2 seconds when excluded successfully and the LED's will be solid red for 2 seconds if exclusion fails.

Your hub should confirm exclusion and the relay will disappear from the interface
Once you receive confirmation of exclusion, try adding the device to the system to see if that solves the issue.
And if that still doesn't help, try these steps next:

Try the exclusion process one more time

Run a factory reset on the unit before trying inclusion again.

If you tried excluding/including from the installation location, try it again closer to the hub. If the second try is successful, the original spot may be out of range of the Z-Wave hub.
