Try excluding the ZAC36 from Hubitat before giving inclusion another try!
You can remove any Z-Wave device, including the ZAC36, on the Hubitat app, even if it was never successfully included - the app should confirm an unknown device was removed. Just make sure you have the latest Hubitat verison installed.
Follow these steps to exclude your ZAC36 Valve Actuator from Hubitat:

Open Hubitat in your Internet browser

Go to Devices

Click on + Add Device in the top right corner

Click on Z-Wave

Click on Start Z-Wave Exclusion

While the device is searching for a device, tap the switch button 3 times quickly - it's a touch button so wait for a beep each time you press it (there will be no tactile feedback like on a push button).

The app will indicate when the device is successfully excluded.
After a successful exclusion, try pairing the Valve Actuator to your Hubitat network again. You can find the inclusion process here.

Try excluding the device again in case the command didn't go through correctly on the first try.

Bring the hub closer or use a hand-held secondary controller (like a Z-Stick) to see if that helps make communication easier between the Valve Actuator and hub. You can also try adding a Z-Wave signal repeater (any non-battery powered Z-Wave device) between the ZAC36 and hub to rule out any range or connectivity issues.
